There's also the option of OTHD if it were truly accidental and not sheer idiocy, but this sounds like manslaughter, which can lead to either a BCD or DD depending on the circumstances.
I don't question a round in the chamber as a sentry, but the normal carrying posture is barrel at the ground or in the air, and finger off the trigger. The only way I can see someone getting hit in the chest is if they were swept with the barrel level, and you never raise the barrel unless you're preparing to shoot.
Maybe there's something else to the story, and I suspect there are plenty of cameras covering the main gate for NCIS to review.
