Pretending I work for US for a second..
There are two different kinds of FMLA, regular and intermittent as far as I know. For example, my heart condition is permanent, and therefore would qualify for intermittent fmla because I would have to miss work due to dr's appointments or problems that could arise at any time. So I call out on the sick line or to admin or whomever and make sure that I am specifying that I'm out using FMLA for myself.
Now let's say that I have, oh I dont know, Peeeeneumonia because I caught it in the hospital while getting treated for said heart condition. I would still be calling out under FMLA, but I would need my dr to fill out two different forms because 1 was for intermittent/heart condition and then another issue arose that still qualified me, but is not a permanent condition. The second FMLA took me out for three more weeks, but the FMLA is only good for those three weeks that I was out. So I would just kind of get permission for being out for three weeks and if it was approved there would be no occurance.
The first FMLA though, if I feel my heart racing any given day I'm supposed to work, or I couldn't move my cardiologist appointment, I can call out and specify that I am taking FMLA for myself/my heart and the FMLA office would know since it's on file and no questions would be asked. If I ended up having to have surgery to correct that, I could call out FMLA in advance for the surgery and still get no occurance.
Of course this is all just hypothetical and it's the way I understand it. You know I dont have any kind of handbook in front of my though so sorry if there are inaccuracies.
