The same way that a mechanical problem turns up in your car after you had it in the shop. It was just a timing thing. He was not placed on the Federal no-fly list until after he had already started training.

The Feds are not going to tell you details about cases they are working or people possibly under suspicion before they are ready to move forward with charges. What if a false accusation had been made against the young man by a bitter ex?
 
There's no real need to modify the training sequence...

When the no-fly lists came out, we (i.e. my rogue programming group) were doing a comparison against the HR databases on a minimum of a weekly basis. That eventually got automated within the confines of SHARP (SAP).

Sounds to me like the system still worked as designed, no?
