Domestic Partner Help

Need some help... trying to access the hub to get the Domestic Partner form to add someone to my flying passes ( for as long as they will last anyways)

Can't seem to find it on the hub anywhere..... anyone know if the form is available online or am I going to have to obtain a form next time I go to work for from CCY??

You should be able to find the forms from the hub ..Go to benefits, then forms and the first section is for domestic partners. There are 7 different forms to download.
